Multiple Sclerosis (MS): Treatment of Neurological Symptoms & Disease-Modifying Therapies

Multiple Sclerosis (MS) is a long-term condition where the body’s own immune system mistakenly attacks the protective covering of nerves in the brain and spinal cord. This protective layer is called myelin, and when it gets damaged, nerve signals slow down or get blocked.

Because of this, MS can cause different neurological symptoms like weakness, numbness, balance issues, fatigue, and vision problems.

While there is no complete cure for Multiple Sclerosis yet, there are excellent treatments available that help:

Let’s divide these therapies into two simple categories:

1. Neurologic Symptoms Treatment

These treatments help patients feel better by controlling or reducing specific symptoms caused by MS.

a) Fatigue management

Symptoms most commonly associated with Multiple Sclerosis include the following: fatigue. Treatments include:

  • Energy-efficient methods
  • Light exercises
  • Sleep management practices

b) Muscle spasm & rigidity

TREATMENT

The muscular stiffening or tightness with Multiple Sclerosis is treated most often with:

  • Physiotherapy
  • Stretching exercises
  • Muscle relaxants – such as baclofen or tizanidine
  • Botox injections in extremely severe cases

c) Pain Management

Multiple Sclerosis can cause either nerve pain or musculoskeletal pain. Treatment options include:

  • Pain killers
  • Gabapentin or pregabalin for neuropathic pain
  • Physiotherapy
  • Relaxation techniques

d) Difficulty Walking & Balance

MS can cause problems with movement, balance, and walking. Assistance includes:

  • Physiotherapy
  • Balance training
  • Walker cane, mobility aids
  • Drugs that improve walking speed, like dalfampridine

e) Bladder & Bowel Problems

Many have urinary urgency problems or constipation issues. Treatments include:

  • Bladder training
  • Dietary Changes
  • Medications
  • Pelvic floor therapy

f) Eyesight problems

Optic neuritis is common in MS. Treatment generally includes the following:

  • Anti-inflammatory steroids
  • Symptoms may improve within weeks.

g. Symptoms of Emotional and Cognition

MS may cause problems with memory, concentration, and mood.

Treatments include:

  • Cognitive rehabilitation
  • Counseling or psychotherapy
  • Antidepressants-if indicated
  • Support groups

These symptom-based treatments do not stop the disease, but they make daily living easier and more comfortable.

2.  Disease Modifying Therapies (DMTs)

These are considered major treatments for MS.

Disease-modifying therapies don’t just treat symptoms—they work on the immune system to slow down the disease itself.

DMTs help in:

✔ Lower risk of recurrence

✔ Delay the onset of disability

✔ Helps in reducing the inflammation of the brain and spinal cord.

✔ Prevent new lesions-a.k.a. damage spots seen in MRI scans

These can further be classified into three major subclasses:

Injectable Therapies

These are given either under the skin or directly into the muscle.

They are generally indicated for early-stage or mild MS.

Oral Medications – Tablets / Capsules.

These are easier to take and are widely used today.

They reduce recurrences and control the activity of the immune system.

c) Infusion Therapies (IV Treatments)

These are given in a hospital/clinic through the drip.

They’re pretty good for dealing with moderate to severe MS.

These treatments significantly slow disease progression and are often used when other medicines don’t work.

During sudden flare-ups, doctors often give high-dose steroids to quickly reduce inflammation and speed up recovery.

Most importantly, disease-modifying therapies help slow down the long-term progression of MS and prevent relapses, making life more stable and manageable. With early diagnosis, regular follow-up, the right medications, and supportive care, people with MS can lead active and fulfilling lives.
